It apparently worked, as he eventually ended up at the Southdown Abbey on Persephone. Book finally found peace tending the Abbey's garden, but knew he couldn't stay there indefinitely. After an unknown amount of time at the Abbey but presumably several years , Book decided to leave and get back out into the galaxy.
The Abbey had access to the Cortex, and Book had tired of watching others suffer along the rim. He announced his intention to carry the Lord's message out to others, and this leads us directly to his introduction in S01E01 "Serenity". As for your specific question, his ident chip reflected his official status with the Alliance - a retired Captain or Commander.
Especially in a medical situation like that, a status like that would require any Alliance officer to render immediate assistance. Shepherd Book was abused as a boy, and lived on the street for a while. He joined up with the independence movement, and volunteered to be a spy on the Alliance.
He was an officer until he led a brilliant defeat, after which he was discharged. He later lived in a monastery for a time, and became the preacher we all know The lead villain in Serenity commanded the respect from the alliance forces that Shepard Book did once they saw his ID.
I wonder if there is still something we don't know about him, perhaps he too was one of the fixers. Henry Evans had a troubled childhood and was a criminal for a time before enlisting with the independence movement. As a member of The Independence he volunteered to spy on The Alliance. He killed a man named Derrial Book assumed his identity to enlisted in the Alliance Military. While still working as a spy for The Independence he quickly worked up the Alliance ranks to become a prominent officer.
At a key point during the war he intentionally lost a major battle dealing a crushing blow to the alliance and leading to him being discharged. This loss was so crushing that the alliance made a point to wipe all record of it out.
Meaning on paper shepherd Book is nothing but a retired prominent Alliance officer. After being discharged he "found god in a bowl of soup" while at a soup kitchen and decided to join an Abby. He eventually left and ended up boarding Serenity. From the episode with the bounty hunter it does make it seem like he was once him self a bounty hunter and then became a preacher.
That would also explain why the alliance gave him treatment. He may have done some bounty work for the alliance, as you all know the bounty hunter does say " that man is not a preacher". I think it's more plausible that he might be former Allience 'non-existing' agent, just like one that was chasing Firefly crew in 'Serenity' movie.. How he became the man he was in the show and who he was previous to that.
It takes you deep into his past. You really get to see the entire evolution of this man. This story is much more character-oriented than adventure-oriented. It is a great ride to be sure, but it doesn't have the consistent action element that the show or the previous comics had. I mean, the guy's a shepherd, and the majority of the comic covers the period before he got on Serenity and started hanging out with those hooligans. He was also an extremely capable fighter and an expert marksman.
The mystery surrounding Shepherd Book deepened even further when it was revealed that he had a high level of clearance within The Alliance. In Firefly 's makeshift series finale, Book is also recognized as a threat and more than he seemed by Jubal Early - with the bounty hunter declaring, " That ain't a Shepherd ".
Serenity served to leave that particular mystery unanswered. Even the character's dying breath was used to deny Mal and the audience the truth of his past. As a result, theories abounded for many years.
The most prevalent of them was that he'd been an Operative of The Alliance, similar to the one played by Chiwetel Ejiofor in the movie. The truth, however, was definitively revealed in the comic book, Serenity: A Shepherd's Tale.
The story revealed that Book had actually been born Henry Evans. Raised in an abusive home, music served as his childhood solace. After running away, however, he fell into a life of crime - making a name for himself as a master thief. Evans remained a career criminal until an armed stand-off with a police officer led directly to him being recruited by The Independence movement.

Magellan , a Tohoku-class cruiser. The commanding officer , after tersely dismissing Mal, changes his attitude once one of his officers shows him Book's IdentCard. Though the exact information on the card is never shown, the crew does note that it affords Book access to the medical facilities on board. On the Browncoat Cruise , Ron Glass revealed with Whedon's permission that Derrial was not in fact the shepherd's real name, but the name of a man he had killed.
Around the time of the Serenity film, fans speculated that Book was an "Operative" before his time, due to his extensive knowledge of how the Alliance would react, as well as the know how of The Operative despite said character theoretically "not existing," believing that the theory was supported by the fact that the Operative has full access to secret Alliance files and classified documents, while Book's identity card affords him priority access to Alliance facilities as seen when he was wounded.
Readers learn in the comic Serenity: The Shepherd's Tale that Book was an Independent fighter who joined the Alliance before the war began, to serve as a mole. He moved quickly through the ranks and was known for his single goal: to end the war by whatever means possible. He was discharged from the Alliance in after an operation he oversaw resulted in a massive ambush and the deaths of 4, people; the Alliance covered up the incident.
